An interprofessional group of health care practitioners, including Family Physicians and Nurse Practitioners provide health care in an integrated, respectful and holistic environment. The focus of the health team is on disease management and prevention, rehabilitation, and health promotion.

We serve adults, children, babies, and families who need health care. We specialize in those with a serious mental illness and are committed to providing the best health care possible.
